In a bid to discover what was done to his sister, Doctor Simon Tam commissions the rest of the crew on the Serenity to rob a hospital of valuable medical supplies while he uses their medical equipment to examine her.

This is the first time for three things:

This is the first time River draws blood in a violent whim.

This is the first time we see Simon's cunning and intelligence in action.

And this is the first time we see how ruthless and desperate the Alliance Agents are when it comes to finding and getting their hands on River.
